If your insurance requires a referral, you must first visit your Primary Care Physician (PCP). Your PCP must submit the referral electronically through your insurance portal if you have Aetna or United (printed copies alone are not sufficient). Ask your PCP for a printed copy of the electronic submission for your records.
All other insurance plans generally accept a paper copy from your PCP. Check with your insurance carrier directly to see what is required per your policy. Additionally, confirm whether your insurance requires prior authorization for specific services prior to treatment, if necessary.
Your referral must include:
- The appointment date or a valid date range (e.g., 01/03/2025 – 12/31/2025). A referral dated after the visit will not be considered valid to submit.
- The number of visits allowed
- The provider’s name (Some insurance plans require scheduling with a specific provider; check whether our group NPI allows flexibility.)
- The reason for the visit (e.g., a referral for acne will not be valid for other services)
- A new referral is required for each new medical concern.

Failure to present a referral at your visit will require you to reschedule your appointment.
If you prefer not to reschedule, you can choose to be a “Self Pay” patient and submit the claim on your own.
